Make a police report (See links on Step Two)
If your passport is lost, stolen or damaged you need to obtain a replacement passport to enable you to remain in the UK and travel internationally.
Upon realizing that your passport has been lost or stolen, please make a report at the nearest police station of the incident. A police report has to be done as soon as possible even though you do not have any urgent travel plans or intention to apply for an urgent replacement passport from The High Commission.
A police station in the UK may request for endorsement of identity from the High Commission in order for a full police report to be produced. If this happens, please obtain the endorsement of identity by attending the High Commission from Monday to Friday (except Public Holidays) from 9.00 am to 12.00 noon (Operation hours may differ due to COVID). Please bring a copy of the lost or stolen passport, MyKad or Birth Certificate (if any).
A full police report has to be in a form of letter/ note/ slip stating the address and telephone number of the police station, report/ crime number, name of victim and lost and stolen Malaysian passport number.
Note:
Damaged passport does not need to be reported to a police station. However, please be informed that certain airlines may refuse to allow passengers to check in with a damaged passport. Hence, please attend the High Commission with the damaged passport for us to evaluate the damage and to advise you further on obtaining a replacement passport (if necessary).

Be self-present at the High Commission in London
Please attend the High Commission in person to report and apply for a replacement passport. The Opening times for report of lost, stolen or damaged passport is Monday to Friday (except Public Holidays) from 9.00 am to 12.00 noon. Please refer to the updated list of Public Holidays of the High Commission. No booking or appointment is required. Applicants will be attended by queue number system. (Operation hours and services may differ due to COVID).
Documents required reporting a loss or stolen passport is as follows:
1. Full police report from UK Police Station (A full police report has to be in a form of letter/ note/ slip stating the address and telephone number of the police station, report/ crime number, name of victim and lost and stolen Malaysian passport number). Due to the change in the UK Government Policy at Police Service Department, they are not providing any more assistance towards individual lost of property in public. In order to adhere to the requirement you are required to log a report onto this website www.reportmyloss.com or www.met.police.uk or www.immobilise.com and print out the report for submission to our office.
2. A copy of completed and duly signed lost passport application form (IM.42c) that can be downloaded from this website. *This form can be handwritten or typed. Please print front and back on same sheet of paper.
3. A copy of completed and duly signed Borang Soal Selidik kehilangan passport dan akuan sumpah that can be downloaded from this website. *This form can be handwritten or typed. Please print front and back on same sheet of paper.
4. Original and one (1) photocopy of original MyKad and Birth Certificate.
Note: If the original MyKad/ Birth Certificate is not available in the UK and is in Malaysia, please seek assistance from your next of kin in Malaysia to have the original MyKad/ Birth Certificate to be certified true copy at any National Registration Department or Immigration Department in Malaysia. A certified true copy is the photocopy of a MyKad/ Birth Certificate that is stamped and signed as a true copy of the original by an authorized National Registration or Immigration Officer. Subsequently have the certified true copy scanned and email/ posted to the UK. Please do not take the risk to post the MyKad/ Birth Certificate as it might get lost in postage.
5. Original and one (1) photocopy of original Citizenship Certificate (if applicable).
6. One (1) photocopy of lost or stolen passport (if any).
7. Original and one (1) photocopy of residency in the UK e.g. UK Long Term visa i.e. Work Permit or student visa or indefinite leave to remain (permanent resident endorsement/ stamp or letter from UK Home Office granting permanent resident status).
8. Passport photo not required. There will be a live image capture at the desk counter once approval process has been granted.
Additional Requirements (By Case)
9. Proof of currently employed by company/ organization – applicable to UK work permit holders e.g. cover letter with company’s letter head (including address and contact details) confirming that the person is currently working with the company. The cover letter has to be recently signed by a managerial/ human resource/ administrative personnel of the company.
10. Proof of currently studying at a university/ education institution – applicable to UK Student visa holders e.g. cover letter with university’s letter head (including address and contact details) confirming that the student is currently studying at the university. The cover letter has to be recently signed by a managerial/ human resource/ administrative personnel of the university. For government sponsored student, confirmation letter has to be issued by the sponsors (e.g. MOHE, JPA, MARA, Petronas, Bank Negara etc)
11. Proof of dependent to a work permit or student visa holder – applicable to UK dependent visa holders e.g. letter from sponsor or photocopy of spouses’ work permit or student visa.
12. Emergency Certificate – applicable to a person with UK Residency who’s passport has been lost or stolen while travelling in European countries and was issued with a temporary travel document by the Malaysian Embassy/ Consulate in Europe to enable return to the UK and obtain a replacement passport.
Processing Times
A lost or stolen replacement passport application will be process within five (5) working days. Processing will only commence upon all required documents been produced. Hence please ensure to submit complete documents when attending the High Commission. Please use the lost or passport replacement passport application checklist to ensure complete submission of documents. The High Commission has the right to request additional documents (if necessary) and arrange further interview session.
In the event of the lost or stolen passport had been found, please notify The Immigration Department, High Commission of Malaysia London as soon as possible.
Urgent Travels
Unless an urgent travel arrangement has to be performed or the lost or stolen passport has been found, please avoid contacting the High Commission for information of decision during the five (5) working days of the processing period. Nevertheless, an applicant is advised not to make any travel arrangements before the decision of an application been made.

Decision
The applicant will be contacted by the High Commission via telephone or email upon a decision been made to the lost or stolen passport replacement application. Upon approval been granted, the applicant will be required to attend the High Commission for biometric thumbprint verification process and collection of a replacement passport. A replacement passport will be charged at the same rate of a renewal passport. A penalty fine will be imposed on those whose passport lost due to negligence.
For cases of an application been refused, the applicant is allowed to appeal to the Immigration Head Office in Putrajaya, Malaysia through the High Commission.

Re-obtain UK Visa endorsement
Upon a replacement passport been issued, please consult the UK Home Office to re-obtain your UK Visa endorsement / stamp that was lost in the missing passport. It is best to travel only after the UK visa been sorted and obtained.
